Boss, I have had same scene. Dealer had recall on sticking throttle, only few days later, all vsc,cle,4wd light up since then. Vent for Inspection- now I have 0138!

At the 4 wire connector FROM the vehicle, one wire has 12v for the heater, guessing 2 other wires are eventual grounds, but isn't the 4th wire suppose to have a reference voltage for the O2 sensor? Something like .3 to .5 volts?

Trying to determine if it's related, but just prior to the check engine light illuminating (a few startups ago) the Rav4 was at the Toyota dealer for a recall regarding the sticking throttle situation, involving a software upgrade and some cable replaced. Unsure if they touched or replaced the ECM.
Vehicle starts and runs fine, no drivability issues.

Installed OEM fit Nav Sys, backup camera!
Then REMOVED all within few months!
= wasted $500 to Chinese economy....my LAST investment on anything Made by China without US Branding.

I agree with EarlMantis....Replace Bank1 Sensor 1, Air Fuel Ratio O2 sensor. I had same issue after replacing O2 Sensor 2. After much trouble shooting, wire harness, voltages, high end scanner, live sensor streaming.. It took care of my problem..4 cylinder..
