If the system is full of mostly water by the 3rd flush, shouldn't you be pouring CONCENTRATED coolant into the radiator so that when it is all mixed it would be more or less 50/50?

No volts, faulty Lambda; volts, faulty gauge. I think widebands give 0 - 5v.

Don't think it's nos. It would have the jets in the runners, or a single jet before the throttle. It would also have high pressure hose to the jets.
Possibly water injection, but still doesn't look...
