My 2018 XLE sometimes doesn't downshift to 1st gear when I stop. I give it gas and it barely moves then huge bang/clank into 1st. The car has 118,000 miles. Is the transmission done or do I just have it flush and filled?

I'd say have it flushed. I know many will say it'll ruin your tranny and to just drain & refill half a dozen times but it sounds like it's on its last legs now so you IMO have nothing to lose.

Try 3x drain and fill and add LubeGard Platinum on the last fill. 120k miles is very early for a trans. to fail even if it's all city miles...Is yours AWD? If so you should change transfer case fluid first.

Also Toyota WS fluid is overpriced, you can use Idemitsu TLS-LV instead which is the same fluid but half the price. You'll need 10 - 12 quarts.

Gear oil for transfer case and rear diff.
