Hello I just brought a 2012 Rav4 from private citizen that I know
And the accelerator issue just happened to me at a stop sign
I looked up recalls and my current one does not have any.
I have had the car 3 weeks
The gentleman I bought it from said it hapened to him two times long ago and he said he must have had his foot on the accelerator
He only serviced his car at a dealer where he bought brand new

To my knowledge/opinion which could be wrong the accelerator issue was floor mats pushing the gas pedal. It's tragic but ridiculous that people actually drove around for minutes with stuck pedals with no thought to put it in neutral, slam it in park, or turn the cars off. I've had cars that actually get stuck accelerator pieces in the engine bay, surge forward with power, and I never crashed them.

The recall fix is to cut the accelerator pedal (let Toyota do this, dont DIY). I highly recommend you get this done for safety if its not done.

I think the fix is also a joke. The floor mat thing is a dumb problem with a dumb fix. It was a problem overblown by the media as people feared technology at the turn of the millennium (eg Y2K). However if my car with an open recall jumped forward I might just get the snip.

This is a sensitive safety issue so I write to be clear: Ignore my experience and opinions. Do all the recalls, be safe, seriously.

Remove the floor mats.

Can you clarify “accelerator issue”? Guessing it got stuck with high revs, but knowing what happened and what resolved it would be a big help.

Otherwise if it was electro/mechanical and not a floor mat problem I would head straight to the Toyota dealer. Have them check for any computer codes, etc. I don’t believe there was a sunset on no-cost dealer repairs for fixing it plus you’ll have documentation if needed in the future.

Repeated testing by NHTSA and Toyota regarding unintended acceleration has not found any fault in any of Toyota's throttle control system, although a few who have posted on this forum dispute those findings. Toyota responded to this potential problem by changing the design of the floor mat, and increasing the distance between the brake pedal and the accelerator pedal on production models, if I recall from 2010 onwards. Also from that production Toyota equipped all RAV4s with Smart Stop Technology. "As a part of the Toyota Star Safety System, the Toyota Smart Stop Technology offers assistance to the driver when braking in the event that the driver unintentionally accelerates. Most commonly, this technology springs into action when both the brake and accelerator pedals are pressed at the same time, as some drivers do when emergency braking." Subsequent "official" blame for unintended acceleration has been that drivers accidentally have pressed the accelerator pedal rather than the brake pedal.

All good info. (y) Depending on what the OP experienced, I would still recommend visiting her local Toyota dealer to check codes and ensure everything is as it should be.
