My 2015 with less than 2,500 miles lost all refrigerant over the winter. The dealer said the problem was a dirty O-ring. It looked as if someone on the assembly line dropped the O-ring on the factory floor and put it on the vehicle dirty. Result was a leak. Was this a one-off or a common problem?

Not common at all. At least the first that I've heard of on a Toyota. There are cases of leaking shaft seals on some compressors - even brand new out of the box compressors can leak at the shaft. But even those are pretty rare. Sounds like you got incredibly unlucky with this one. Possible that the o-ring was faulty in the parts bin, or the car was assembled on a Friday.

^ What he said! I had an '09 RAV and now I have a '15 XLE, in all the time that I've been on this forum your's is the first incident that I've read about dealing with HVAC O rings.
