The fuse is located in the interior fuse panel near the drivers left knee.

It is a 15A fuse (blue in color) and it is labelled RR WIP

Not the fuse.

The rear washer stopped working. Where (exact location) is the fuse for the rear pump motor?

Any idea what I need to do to remove the reservoir?
Does it come out from the above or from below?
That would be great help.
Rock Auto has the reservoir and the two pumps for about $20. (Not Toyota.)

Could it be caused by a plugged or broken tube?. Can you hear the motor humming when you operate the switch (you might need a helper for this)? Do you see water running on the ground after operating the switch?

It could be a defective switch. You could find the switch connector near the steering column and test the switch with a multi-meter. You could also access the bottom of the reservoir and reverse the connection to the two pumps before deciding if a new pump is required.

I can't really answer that. This PDF may help, but I'm not even sure what model year those instructions are for. Additional searching on this site could produce more results.
