I've had an issue with my 2008 Rav4 2.4L Sport for about 2 years now. I first noticed the problem when going up a ramp to an elevated roadway that the engine suddenly lost power and would not accelerate. The Check Engine, VSC, and Anti Skid lights came on and stayed on. The engine just loped without being able to accelerate beyond 30 MPH. After shifting to a lower gear (which revs the engine) it will usually allow the car to resume the regular power level with normal acceleration. However the lights stay on which renders the cruise control inactive. (I know this is a safety feature). The Check Engine code says that I have a Cylinder 4 Misfire.

The dealership changed all four ignition coils and all four plugs. The engine ran normally for about 2 weeks and then the same issue happened. I returned the car to the shop and they changed the computer. Once again it ran well for about 2 weeks and then the same problem started. With $2400 now invested, naturally I am concerned about anymore trips to the service department.

After some internet searches, I learned that it would be good to recheck the plugs and gaps. I did so with no changes to the problem. I also decided to swap the ignition coils around to see if it would show a different cylinder misfiring, but it still only shows a misfire code for the #4 cylinder. I checked the harness to the coils (especially the routing to the #4 coil) and all the wires appear to be clean and intact. I've run some injector cleaner through the system thinking that this may help with the fuel/air mixture.

Finally I broke down and decided to drop the fuel tank and replace both the in-tank fuel pump and filter. Still have the problem. When the car runs on flat ground in normal operation, it runs fine (even though I have noticed a slight drop in fuel mileage/economy). Yes, I've even changed the fuel cap for the possibility of a faulty seal. The problem still rears its head under the load of an on-ramp climb.

I'm not wanting to just start swapping parts in hopes that I'll get lucky. I'm simply hoping that someone can steer me in a direction that makes sense to try before settling on scrapping this old car. Any suggestions?

Based on general knowledge (not specific to the engine in question), my guess is if you are getting a misfire always on cylinder 4, the first thing I would check is to see if the spark plug for that cylinder is fouled. Does it appear black and wet as opposed to light gray? Does the engine have an oil usage issue (more than say a quart every 1000 miles)?

If it looks ok, then I would look for a valve problem in that cylinder. It could be a valve not sealing properly or it could be related to a valve not opening adequately. (e.g. burnt valve, bent valve, or worn cam).

All four plugs looked nearly like new when I pulled them to recheck the gaps. No fouling at all.
I was wondering if there may be some issues with valves on that cylinder, but I assumed there would be similar symptoms at times when I was driving level as well. It still has a lot of pep in 'normal' driving. My car has a shade over 155,000 miles on it which is low for a 2008 model. However I do know that a burned valve can happen just about any time.
 
I am also wondering if it could be the catalytic converter going bad. My daughter recently had some sluggish acceleration issues going on with her car, but no dashboard lights turning on. Turned out to be a bad cat and acceleration was back to normal with the new one installed.

Intake manifold gasket? I'd give it a preference over injectors or a cat converter.
Would such a problem create the misfire error code P0304 every time the issue occurs? Would the engine run well while it is on a level surface? It would seem that an intake manifold gasket would have the engine running bad all the time.
 
Gaskets usually do not fail on the whole perimeter. In your case the air may be sucked around the 4th cylinder leaning the mixture to the point it cannot ignite.

The loss of power would be noticeable only when the power is called on engine, e.g. going uphill or a maniac acceleration from a stop sign. In other cases even 2 cylinders can provide sufficient power. Look for a Youtube video on cylinder deactivation in Mazda CX5. It has a nice graphical output on working cylinders. I am not a fan of CD, but the video is educational.

Fuel Injectors. For all trouble I've had so far with my injectors, I've never experienced a power loss. Coughing and delayed start - yes, fuel drinking - yes, but not loss of power. I've never removed injectors or a fuel tank. Techron injector cleaner and avoiding certain gas station worked so far. I even bought a set of new injectors, but have not seen a need to replace them.

Cat Converter. I have not heard about a clogged one, but I am not a muffler shop to have sufficient exposure. Cats usually set a code. I am aware of one with broken internals though. That one was dinging so loud on cold start, that neighbors were swearing. However, nobody complained about loss of power.

I am not saying that the intake manifold gasket is warranted. I am saying that it would be the first thing I would check if power loss happened to my car.
 
Same exact issue on a 2007 2.4 Engine, random Cylinder #4 Misfires during sudden acceleration or high power demand, particularly when going uphill. Tried with everything you did. Replace spark plugs, and then coils, used fuel system cleaners at least 4 times. Final fix was replacing all fuel injectors. All of them with heavy deposits but number 4 was the worst, never performed ultrasonic cleaning. Everything improved dramatically, ST and LT fuel trims are back to normal, acceleration and response is back. Next step is to replace fuel pump and filter to reset the entire fuel system.
